Fidelity Checklist: Botvin LifeSkills Training (LST) Middle School Levels 1–3

These documents
describe the fidelity monitoring process for Botvin LifeSkills Training (LST), an evidence-based,
classroom-based universal prevention program listed on the Blueprints for Healthy Youth
Development registry and designed to prevent adolescent tobacco, alcohol, and marijuana use, as
well as violence. The Level 1, 2, and 3 fidelity checklists provide structured observation tools to
assess whether each stage of the curriculum is delivered as intended. Across all levels, the
checklists guide observers in rating adherence to core lesson components, facilitator practices,
and student engagement, supporting consistent, high-quality implementation and continuous
improvement across the full middle school sequence.

Fidelity Checklist: Strengthening Families Program 10-14 (SFP 10-14) – Guidance For Conducting Fidelity Observations

This document outlines the fidelity monitoring process for SFP 10–14, a family-based prevention program listed on the Blueprints for Healthy Youth Development registry that reduces behavior problems and substance use by strengthening parenting skills, parent–child relationships, and family communication. Fidelity Checklist: Project Towards No Drug Abuse (TND)

This document outlines the fidelity monitoring and assessment approach for Project Towards No Drug Abuse, a high school, classroom-based prevention program listed on the Blueprints for Healthy Youth Development registry that aims to prevent adolescent use of alcohol, tobacco, marijuana, and other drugs. It guides observers in systematically assessing how closely the curriculum is delivered as intended, using itemized rating scales for lesson adherence, facilitator delivery quality, student engagement, and overall session execution. The checklist also includes space to document notable “change” events or “wow” moments that capture meaningful participant responses or indicators of high-quality implementation.

Project Management: Responsible, Accountable, Consulted, Informed (RACI) Diagram

Any small and large project involves multiple individuals and stakeholders. It is extremely important in managing tasks across the project life cycle to define which individuals/stakeholders are Responsible, Accountable, Consulted, and Informed (RACI). A RACI Diagram or RACI Matrix can assist program managers in identifying who needs to be involved and how they are involved. You can view this video for an overview of RACI Diagrams.
